Gender inequalities in glucose care and control in type 1 diabetes in Germany and France: findings from DPV and SFDT1

Background and Aims: We aimed to evaluate whether diabetes care and glucose control differ in men and women with type 1 diabetes (T1D) in two European countries.


Methods: We compared glucose data in adults with T1D in the DPV (n = 11,392, Germany, 17.1% university hospital, age 32 ± 17 years, diabetes duration 18 ± 14 years; 53.7% men) and SFDT1 studies (n = 1,259, France, age 40 ± 14 years, diabetes duration 23 ± 14 years; 54.2% men, 89.2% university hospital) in 2020‐2022.


Results: The use of continuous glucose monitoring (CGM) was 67.1% in DPV and 90.1% in SFDT1. More women than men were CGM users in DPV (69.1 vs 65.5%, respectively, p < 0.001) but not SFDT1 (90.0 vs 90.2%, respectively). In both countries, women were more prone than men to use continuous subcutaneous insulin infusion (DPV: 43.1 vs 32.0% respectively, p < 0.001; SFDT1: 29.3 vs 21.1%, p < 0.001); low‐glucose suspend (DPV: 12.1 vs 10.0%, p < 0.001; SFDT1: 17.7 vs 12.0%, p < 0.001) and hybrid closed loop systems (DPV: 9.7 vs 7.0%, p < 0.001; SFDT1: 9.0 vs 5.2%, p < 0.001). HbA1c levels were slightly lower in women in DPV (8.0 ± 1.7 vs 8.1 ± 1.9%, p = 0.009) while we observed comparable results in SFDT1 (7.9 ± 1.4 vs 7.8 ± 1.4%, p = 0.33). Time in range was higher in women in DPV (55 ± 18 vs 53 ± 19%, p = 0.045) with similar percentages in SFDT1 (56 ± 17 vs 58 ± 16%, p = 0.156). Time below range was similar in women as men in both countries.


Conclusions: In Germany and France, women appear to be more likely to use CGM and modern insulin delivery systems with better glucose control in women than men only in Germany.
